LIVESTRONG Survivorship Notebook 
Bulk Orders

Thank you for your interest in ordering a bulk shipment (20 or more) of the LIVESTRONG Survivorship Notebook from the LAF.

The notebooks are free, however shipping and handling charges do apply.

Here is how the process works:

1. Complete the form below and send to the LAF.
2. See the chart below for shipping costs associated with your bulk order.
3. After you approve the shipping cost, we will process the order.
4. An invoice will be sent to you.  Please note, invoices are due within 30-days of receipt.

You should receive your bulk shipment in approximately 2 weeks of confirming the order.

Orders under 100 notebooks will be sent 10 per box.  Each box weights approx 40 lbs.

Orders over 100 notebooks will be send on a pallet. There is an additional fee of $35.00 if no loading dock is available. 

There is no limit to the number of notebooks you may request.  Each notebook weighs 4 lbs.
